What is MyChart?

MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:

  • Communicate with your doctor's office.
  • Request medication renewals.
  • View your lab results released from your physician.
  • Communicate electronically and securely with your medical care team.

How do I sign up?

You may request sign-up for MyChart in one of the following ways:

  • Request sign-up during an office visit.
  • Call the MyChart staff at (608) 324-3742 between the hours of 7:00 AM and 5:00 PM Monday through Friday.
  • Send an e-mail message to mychart@monroeclinic.org. Include your full name and date of birth.
Once we receive your request, we will send access instructions and a personalized, secure access code to your home address via U.S. mail.

Why do I need my Social Security Number on file at Monroe Clinic to use MyChart?

Your social security number is the one piece of confidential information which will remain constant throughout your lifetime. To ensure the security of your MyChart account, we use your social security number as a means to link your MyChart account with your medical information. Your social security number is necessary if enrolling in MyChart from home or in the event a reset of your user name and/or password is needed. Using your social security number to confirm your identity will ensure your MyChart account remains secure.

When can I see my test results in MyChart?

Your lab results are automatically released to your MyChart account within 48 hours after being resulted if not released sooner by your provider. The automatic release begins at the point of your MyChart Activation.

If some of my health information on MyChart is incorrect, what should I do?

Your MyChart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

If I send a message to my doctor or nurse, when can I expect a reply?

You can expect a reply within two business days. Please note that MyChart should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

Can I view a family member's health record in MyChart?

Proxy access allows you to sign in to your own MyChart account, and link to another patientís MyChart record. Proxy access is available both for adults and for parents of children under 18 years of age; however, access is limited for children age 12-17. The State of Wisconsin limits parents' access to medical information for children in this age group. Adult patients and their proxy must sign the Adult Proxy From, and parents must sign the Child Proxy Form to allow access. If interested, please ask to sign up at your providerís office.

Can I request information about a family member from my MyChart account?

MyChart offers direct access to your personal health record and communication about another individual's information would be placed in your health record. As this information would not appear in the correct health record, it could potentially jeopardize medical care.

I forgot my password. What should I do?

To reset your MyChart password, click on the "Forgot password" link on the sign-in page. Complete your personal information and answer your security question. To keep your account secure, if you are unable to provide the personal information or answer your security question, you will need to present to any Monroe Clinic location with a picture ID. After verifying your picture ID, they will be able to call the Helpdesk to reset your password.

Can you send me a new access code if I have lost it, let it expire or did not receive it?

You can request a new access code at your next office visit, by calling us at (608) 324-3742, or email at mychart@monroeclinic.org and a new code will be sent via U.S. Postal Mail.

Log into MyChart and from the left menu, go to the administrative section and select the appropriate option.

How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal ID's, and passwords. Each person controls their password, and the account cannot be accessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

What is your privacy policy?

MyChart is owned and operated by Monroe Clinic and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by Monroe Clinic.

I was logged out of MyChart, what happened?

We strive to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 10 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

What do I need to use MyChart?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer).

My access code does not work, what should I do?

For your security, your access code is temporary, expires after 60 days, and is no longer valid after the first time you use it. If you still have problems call us at (608) 324-3742, or email at mychart@monroeclinic.org.

Is my access code my user ID?

No, your access code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. (The code will expire after you have used it or after 60 days). When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.
